Parents Information: ENROLLMENT
The center is licensed to accept children age 6 weeks to 5 years whose parents work or attend school. With the above limitations, our center is open to all children regardless of race, nationality, creed or sex, whom we might appropriately serve.
A $50 deposit is required at the pre-enrollment visit. This holds the child’s slot for 2 weeks and will be applied to the first week’s fee. Should you withdraw your child and wish to reenroll him/her, another $50 fee will be charged for re-entry.
An enrollment interview conducted by the Director is required prior to admission. It is beneficial for your child to visit at least once prior to beginning care. Also, for your child's safety and to comply with State regulations, the following forms must be completed prior to enrollment:

2. Immunization Form (filled out by doctor or health dept.)
3. Health History Form
4. Child Care Food Program Form & Addendum
5. Income Verification (current check stub)
6. Payment/Policy Agreement & Emergency Evacuation Form
7. Project Ready for School Form
8. Consortium Consent Form
9. Influenza Notification
10. ASQ3 $ ASQ-SE2
It is necessary that all records be kept current. Please notify the office immediately of any changes in address, telephone number, employment or other pertinent information.
